Physicals! Cheer and Football! July 10th!

Physicals will be given by the league doctor in the P.E. 2 classroom located near the boys gym (10am – 1pm). At least 1 parent or guardian must accompany all players/cheerleaders. The price for the physical is $20.00. You may have your own Physician do the exam but will need our physical form completed and returned by the 1st day of practice (no physical-no practice).

As well as physicals, we have equipment fittings and will be collecting paperwork for player and cheerleader certification. For equipment fitting info please click the “Football’ page. For player and cheerleader certification please bring 1 copy of at the following documents:

• Birth Certificate or Passport
• School Report Card 2009/10. (No progress reports)

All players must show proof of residency. Proof must show the players/cheerleaders current address and parents name. Please provide 1 copy of any 2 of the following documents:

• Utility Bill (electric, gas, or water) NO phone bills will be accepted.
• Cable Bill
• Bank Statement
• Credit Card Statement
• Drivers License
• Paycheck Stub
• Tax Document
